Therefore CHATBOTS should not pose any barriers.

In the wake of CORONA pandemic there is a renewed interest on telemedicine.  Because face to face consultation for phlegm symptoms is not allowed in Sri Lanka. And also there is screening for COVID-19 and also screening for high risk suspects going on. All these can easily be done through the CHATBOT technology with great convenience

Initially we at BIOSOFT plan to release 2 CHATBOTS dealing with CORONA PHLEGM SYMPTOM SCREENER and CORONA QUARANTINE SCREENER.  These will be followed by PHLEGM SYMPTOM ANALYZER and CORONA THOUGHTS APPLICATION.

ALL THE CHATBOTS BIOSOFT DESIGNS ARE FOCUSED ON GIVING A PERSONALIZED DECISION. We at BIOSOFT do not believe in dispensing medical information whole sale and static and most of the time has no personal value. So that is what is call information overload with resultant information pollution. The diet prescription is generated using a software program developed by the author. The program is named as “Suwa Posha”. The program takes an input of 5 parameters. They are weight, height, age, sex, body size, activity levels, food preferences and the diet type. The diet types mainly used for this study are normal diet, diabetic diet, DASH diet, Mediterranean diet and very low calorie diet (VLCD). With this input the program writes a diet prescription which

1) Portion controlled meals
2) Sri Lankan foods
3) Freely available Sri Lankan foods
4) Many healthy eating patterns suggested by various authorities are incorporated (number of fruit servings, number of vegetable servings, regular and spaced out meals, calorie balance, nutritional balance)
5) Inclusions of user preferences
6) Changes of macronutrient compositions of meals – depending on the medical problems co-existing
7) Changes in the calorie deficits in the meals
8) Emphasis on 6 meals per day without skipping any meal
9) Incorporation of emerging evidence in dieting – mediterranean diet, OMNI heart diets
10) Avoidance of inappropriate food fads
11) Implementation of new findings on human nutrition into your meal plan right away :
a) new ideas of diabetes management – vegetarian diets, adding sugar and sweets witin daily calorie balance

1) Identification of the client by the author – first consultation or the subsequent consultation
2) Required measurements are recorded by a trained practice nurse – for every patient in every consultation
3) The measurements taken are – height in centimeters, weight in kilos, neck circumference, arm
circumference and waist circumference in centimeters
4) In case of follow up consultations the same nurse records all the measurements again
5) Repated measurements of height is used to assess the accuracy of the other measurements
6) Also the changes in the anthropometric values are correlated with the patients’ perceptions
7) These are evaluated by asking the following questions :

a) Do you feel the less heavier than few weeks back – usually felt within 1-2 weeks
b) Do you feel the waistline of your dress getting loser
c) Do you feel the chest and back of the chest getting thinner as felt by loser T-shirts and jackets
d) Do you feel the neck getting thinner as felt by less tight collars and ties
e) Do you feel the upper arm getting thinner as felt by the shirts and jackets getting loser

The answers to these questions guide us as to the fat loss and weight loss more accurately than the weighing scale and waist line measurements – reasons for this are weighing scales are not  scientifically accurate, anyway there are day to day changes in the weight which is a common
thing particularly in females.

8) Prescription generation using the web based service of the hospital – a menu plan of foods for a day to be taken for a week is generated. See the appendix for a typical menu plan.
9) Patient is given the following instructions :

a) A menu plan for 1 week given
b) Advice on how to get a 300 ml cup and 15 ml tablespoon
c) All food measurements are for cooked foods
d) Preparation method irrelevant – prepare food tastily, advised to have a wholesome meal
e) Portion size emphasized
f) Same menu plan for 1 week emphasized
g) Every 1 week menu plan is changed and a new menu plan for next week given
h) Every 10 th visit threshold diets are prescribed to challenge the body adaptations
i) After 10th diet food substitution is encouraged based on the already prescribed diet sheets
j) Plateau break diets are prexcribed as and when necessary
k) Exercises are not a part of this diet service and actively discouraged in very low calorie diets
l) Exercises are necessary for many other health benefits

9) Appointment for follow up in week’s time (56) – patient is told the following facts – weight
reduction is achievable, fat loss is achievable, pre-diabetes and pre-hypertension are definitely normalized,
repeated follow ups until targets are essential, after several weeks a nutritional rehabilitation is taking place (see the appendix for definition of this nutritional rehabilitation), steps in food exchanges (Appendix)
10) Follow up targets – more than 3% weight loss, more than 3% fat loss, normalization of blood sugar in pre-diabetes, normalization of BP in pre-hypertension, achievement of target blood sugars  in DM, achievement of target BP in HT, achievement of any other patient directed targets

10) These meal plans will achieve weight loss and waist loss both – during the dieting program sometimes weight is lost, sometimes waist is lost, sometimes both lost – but it surely will occur. We have achieved weight losses upto 40 kg with dieting alone
11) No need of exercise for weight loss during this dieting program –

EXPERT SYSTEMS

Expert systems are computer programs with the following features : input by the user, knowledge base, inference logic and the output by the system.  This site has many medical expert systems developed for primary care.  They basically take the user input as the  program input.  Then the knowledge base is inside the  program.  The reasoning algorithm also is inside the program.  The user input is manipulated inside the program and the output is generated.  The program also is able to justify its output in terms of reasoning  acceptable to the user.  One of the main expert systems we have developed is called CAMEOS.  CAMEOS stands for Computer Assisted Medical Evaluation Of Symptoms in Primary care.

COMPUTERIZED DECISION SUPPORT SYSTEMS (CDSS)

Biosoft CHATBOTS are a series of products simulating the Computerized Decision Support Systems known in the medicine and medical informatics. Computerized decision support systems (CDSS) are health information technology (HIT) based processes to aid health care decision making and action taking.  They have the following typical features:
  • Input – patient data
  • Knowledge base
  • Output – actionable clinical information
For instance, simple BMI calculator can b considered a CDSS.  You have to input patient weight and height to the system.  The formula for BMI is the knowledge based the program requires to make the calculation.  Output is BMI on which you classify the patient as normal or overweight or obese. CDSSs can again be conceptualized as comprising CDSS simple and Expert Systems. The difference between an expert system and a simple CDSS is that the former has a reasoning routine attached to the CDSS.  So that the program itself can inform the user WHY it came to that decision and why does it advise the action it is proposed?

Your sole right for the payment you make in return for the Services given to you by way of any Biosoft product is purely limited to the use of the product to get the answer to the clinical question you pose to the system. Your responsibility is to restrict your use of the service for this right the Biosoft grants to you for the payment you make to the Biosoft.

The use of all Biosoft products are limited to getting an answer to the user questions posed by the user. User questions are produced in terms of the required knowledge to produce the correct answers by the systems itself. You agree not to hold Biosoft accountable for actions, behaviors and conduct resultant from the use of the Biosoft services. While we guarantee the accuracy of the algorithm underlying the service offered through each Biosoft product we retain the right to reject any claim of inaccuracy, fault resultant from the use of the Biosoft product. We also retain the right to reject any claim of physical or mental damage after using a Biosoft product

The users should not engage in the development of new works built upon the package (derivative works).It is strictly prohibited to intercept the messages to and from the user and the server and also to retrieve the server contents of the program by any means. Biosoft has the right to terminate and or to prevent any user using the products the company produces

Intellectual property: You agree that all the content in the Biosoft products is protected by Intellectual Property and other laws including but not limited to copyright. You agree that you will not use such proprietary information or materials in any way whatsoever except for the use of the services for personal use in compliance with this agreement. No portion of the services may be reproduced in any form or by any means except as expressly permitted by this agreement. Any dispute or claim relating to the use of Biosoft products will be resolved by binding arbitration rather than in court. User may opt out of the arbitration within a 30-day period from the date of starting of the claim. WE ALSO HAVE TAKEN INDUSTRY SRTANDARD SECURITY, PRIVACY AND CONFIDENTIALITY MEASURES IN THE DEPLOYMENT OF BIOSOFT PRODUCTS.

Our Service is provided “as is”, and we cannot guarantee it will be safe nor secure nor will work perfectly all the time. TO THE EXTENT PERMITTED BY LAW WE ALSO DISCLAIM ALL WARRANTIES, WHETHER EXPRESS OR IMPLIED, INCLUDING THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE AND NON-INFRINGEMENT.

We also do not control what people and others do or say and we are not responsible for their or your actions or conduct (whether online or offline) or content ( including unlawful or objectionable content). We also are not responsible for services and features offered by other people or companies even if you access them through our Service.

Our responsibility to anything that happens on the Service (ie liability) is limited as much as the law will allow. If there is an issue with our Service we cannot know what all the possible impact might be.
